Parental Consent Forms

Consent forms to be completed 

Please complete each form for each child

These are renewed on a yearly basis in the Autumn term to ensure information and responses are up to date. These consents apply to walking in the local area, photos, videos, communication Google Drive and acceptable use policy for the internet in school. It is really helpful for class teachers as the time taken to chase consents affects the time they have for planning and preparation of lessons.

Y5 and Y6 only - Walking home 

Some children in Y5 and Y6 have written permission to walk home at the end of the day at 3:05pm. If  written permission is received then class teachers issue a 'walk home pass'  

Some Y5/6 children also ask to walk home after clubs finish, so we would like to clarify with parents  that they give permission for this later time in addition to the 3pm finish. In order for teachers or  sports coaches to allow children to walk home they will need their usual pass (issued by the class  teacher) AND for Y5/6 parents to complete this form. This information will then be shared with class  teachers and staff leading clubs. 

If you have more than one child in Years 5 and 6, please complete the following form for each  child.

Children who do not have their walk home pass at the end of the day (or a handwritten note from a  parent or class teacher) will not be allowed out of the gates until permission has been verified with  either their class teacher or by the school office ringing home.
